Dear Department of Human Services,

Can I please have a copy of:

- Any complaints/feedback/correspondence to the department from licensed medical professionals regarding the short and long term health outcomes of their patients in regards to immunisation and the removal of vaccine exemptions

- Any correspondence to the department in relation to licensed medical professionals having their ability to write valid vaccine exemptions removed

- Any internal discussion in relation to the above

To assist the Department, I specifically exclude:
- Duplicates of documents (I only require 1 copy of each document)

Where possible, I would prefer that this information is provided in it's original format - that is to say, either in Microsoft Word or HTML (where received online or via email). I ask that you do not use PDFs wherever possible as it makes it difficult to read the information.

Please deal with this request informally or in accordance with your Administrative Access scheme if practicable. If this isn't practical, please consider it a formal application under the Freedom of Information Act.

I refer to your freedom of information (FOI) request for the following
documents:

 

- Any complaints/feedback/correspondence to the department from licensed
medical professionals regarding the short and long term health outcomes of

Assistance to revise your request

 

I have undergone initial consultations with the business area responsible
for immunisation programs and it appears your request may be too broad to
process.

 

I have been advised that the following terms require some further
specification to be able to properly identify the documents you are
seeking:

 

·       ‘short and long term health outcomes’ – Please advise whether you
are seeking documents that contain this specific phrase or if you are
seeking documents from medical professionals referring to their patients’
health generally. 

·       ‘immunisation and the removal of vaccine exemptions’ – Please
advise if you are broadly referring to correspondence relating to
immunisations, or the particular ‘immunisations medical exemption’ as set
out here:

·       ‘having their ability to write valid vaccine exemptions removed’ –
Please advise if you are referring to a specific phrase you would like the
correspondence to contain, or referring to a specific change to the
immunisations medical exemption.

·       ‘internal discussion’ – Please specify whether you are seeking
discussions held within the business area responsible for immunisation
programs only.

 

It would assist the department in processing your request if you provide
further clarification and revise your request. You may wish to do the
following:

·       clarify the terms above; and

·       specify a date range, for example the last 12 months. (i.e. 6
April 2018 – 6 April 2019). Please note we cannot process requests that
extend beyond the date of the original request (6 April 2019).

 

Please respond to this email revising your request for documents by close
of business Wednesday 17 April 2019. If we do not receive a response from
you by that date, I will send you a formal consultation notice.
