Dear Australian Bureau of Statistics,
This is a request to have the following information released as a Freedom of information request.
1. Sample copy of 2021 census paper form (as sent by post to members of public) including all instructions how to fill out the questionnaire
2. Sample copy of 2016 census paper form (as sent by post to members of public) including all instructions how to fill out the questionnaire.
In the event, paper forms are not available , an online equivalent will also suffice. Please release this in pdf format, even though questions asked are available on your website.

the Freedom of Information Act 1982 (FOI Act)(ABS reference FOI 202122/45)
regarding:

Thank you for your reply. Please release the 2016 census form under freedom of information even though it available on your website.
In relation to the charges, please waive the charges as it relates to 2021 census form which is a very easily accessible document for the ABS.

We have some questions regarding the scope of your request.
1. The ABS produces the Census in various languages including braille. Does your request include all languages or only the English form?
2. The ABS has several forms within the Census. The household form is the main form used but there are also personal forms. Your wording implies a singular form, can you confirm which forms your request includes?
