Thank you for your email.
There are a number of requirements under the SA Freedom of Information Act 1991 which applicants must meet in order for an application to be considered valid. The online application form can be found at https://www.sa.gov.au/topics/about-sa/go....
There is an application fee of $39 or if you have a current concession card the fees can be waived.
Under Section 13(d) of the FOI Act, we would also require additional information to be able to locate the document/s you are requesting.
Your email states that you are seeking:

document to be identified; and
(e) must specify an address in Australia to which notices under this Act should be
sent; and
(f) must be lodged at an office of the agency, and may request that access to the
document be given in a particular way
