Dear Melbourne Health,

I am writing to you regarding the matter of any and all of my personal and legal medical images and or notes related to my circular saw injuring during late of November 27th-30th 2021, including the CT scan taken prior to my operation as-well as the individuals/doctors and or nurses involved in my operation and any individuals/doctors and or nurses that were inbolved in the strapping of my splint post my operation.

I am also aware that there were CCTV footage of the operation and the strapping of the splint, I am also requesting that content as well.

This has now become a legal proceeding and by law you are required to release any and all documents even if it weren't.
