Dear Kim

The NSW Ministry of Health does not hold any medical health records.

Access for medical records is processed under the Health Records and Information Privacy Act 2002 [HRIPA].
https://www.legislation.nsw.gov.au/#/vie...
You will need to directly contact each facility (public or private hospitals, GPs, allied health, specialist services and clinics) for each treatment and admission.
You will be advised if you need to complete a form, provide identification and pay a fee.

Please note that under the State Records Act 1998, records are only held at the facility for certain periods of time.
Under the Act, records are then moved to an off-site archive storage and may be destroyed after a further period of time.

If you cannot find the specific facility you are looking for (some may have closed or be renamed), please contact again since we have some information about where to contact for old public facilities.
Please note that NSW Ministry of Health only has information for public services such as public hospitals, mental health services and community health services.
