Dear Department of Defence,

If possible, please treat this as an administrative request, otherwise consider this a formal request under the Freedom of Information Act, 1982.

I require the following document(s):

- Any record that shows the number and percentage of Defence Members across all three arms (Army, Navy and Airforce) who are deployed to operations overseas WITHOUT submitting the mandatory National Welfare Coordination Centre (NWCC) form. This form is supposedly a condition of deployment.

I ask that any fees be waived in relation to this request: it is in the Public interest to know that their family members may be deployed to theatres of war without completing this *mandatory* requirement, thus denying them knowledge of, and access to, available and essential support services.

"The National Welfare Coordination Centre (NWCC) does not produce or have a report that contains information on members of the ADF who are deployed and have not submitted form AC989 - 'Pre-deployment Emergency Contacts Confirmation form."

Therefore I would like this to be turned into a formal request and would like Defence to PRODUCE a document in accordance with Section 17 of the FOI Act (1982), specifically S17 1 C (i):

the agency could produce a written document containing the information in discrete form by:
(i) the use of a computer or other equipment that is ordinarily available to the agency for retrieving or collating stored information.

I refer to your correspondence, dated 11 March 2018, in which you sought access, under the Freedom of Information Act 1982 (FOI Act), to:
