Laoratory results, report and correspondance regarding GLORICIDE and/or the mixing of 2,4-D and Metsulfuron methyl
Dear Queensland Department of Health,
The Queensland Ombudsman claimed in a report dated 17th December 2012 that Queensland Health's Forensic Scientific Services Laboratory undertook recent testing on the manufacture of GLORICIDE, an Unregistered Chemical Product developed by the SCRC.
The Study in question was commissioned in 2012, possibly late 2011. Correspondence on this report may have continued into 2013 and beyond.
The Ombudsman claims that 2 byproducts of the chemical reaction were identified, i request the Original un-amended Laboratory Reports/Results/Studies undertaken and all correspondence including emails, phone calls, letters, reports, meeting minutes, ministerial notifications, subsequent amended laboratory reports and all correspondence associated with it as per above.

To access these records you will need to apply under the Right to
Information Act 2009 which will require the payment of a mandatory
application fee of $46.40. It is a compliancy requirement under s24(2)(a)
of the RTI Act that you submit your application in an approved form. As
such, I have attached a copy of the Right to Information and Information
Privacy Access Application form (the form) for you to complete and post
back to this office, along with your cheque or money order (made payable

Dear RTI-Privacy,
You have previously denied having the document requested even though the Queensland Ombudsman claims that they commissioned the report from you. Do u really expect me to pay for a report that you deny that you have?
I suggest you provide this report to the APVMA as it is required by law to notify them of synergistic chemical reactions. Your delay has cost lives and this information has been withheld from numerous senators and Senate Estimates Committees and is a Criminal Offense punishable by Jail.

I note the following background to this matter:
1. On 13 March 2017, you sent an initial email to this unit requesting access to certain information held by Department of Health (Forensic Scientific Services)
2. On 16 March 2017, our office wrote to you advising how to submit an RTI application and stating that an application fee would be payable for this application
3. On 22 April 2017, you wrote to this unit, as per below, with an 'internal review of right to information request'
This unit processed your initial email as a query, rather than a formal application, therefore there was no formal application considered to be on foot. I apologise if this was not your understanding. Our email of 16 March provided details of how to submit a compliant application to the Department.
If this matter was processed as an RTI application, I note that the 20 day business timeframe to seek review would have been 13 April 2017, therefore it was outside the legislative review period in any case.
