- Credit Card Payment Form
Once a completed application form and the statutory fee of $30.10 is
received, processing of your application may begin. Should you be entitled
to a fee waiver on the basis of hardship, please attach evidence of this,
typically this would be evidence of a Commonwealth entitlement, such as a
Health Care Card, Pensioner Card or Veterans Affairs card.
If you have any questions or wish to discuss your application please
contact me.
