The NDIA’s formal fraud-risk assessment conducted in March 2015

Please provide a copy of the “formal fraud-risk assessment” conducted in March of 2015, cited in the NDIA Annual Report 2014-15 [1].
Context:
Bruce Bonyhady AM (Chair) stated in his Letter of Transmittal to the Minister of Social Services that “I am satisfied that the Agency has prepared a fraud risk assessment and fraud control plan and has in place appropriate fraud prevention, detection, investigation and reporting mechanisms that meet the Agency’s needs and comply with the Commonwealth Fraud Control Guidelines” [2]. It seems this assertion is supported by the conduct of “a formal fraud-risk assessment in March 2015 as part of the Agency’s fraud prevention, detection and response strategy” [3].
