I am requesting access under the Freedom of Information Act 1982 (FOI Act), to the following information:

1. The contents of the “Outcomes Questionnaire” including all of the questions and any selectable or suggested responses. These may also be referred to as the “Short Form Outcomes Framework”, “Short Form Outcomes Form”, or “SFOF”.
There are at least four variations of the Outcomes Questionnaire, with a different version for:
* young children (before starting school)
* children who have commenced school up to age 14
* young people aged 15-24
* adults aged 25 and over
2. The contents of the “Family Questionnaire” including all of the questions and any selectable or suggested responses.
There are at least four variations of the Family Questionnaire, with a different version for:
* young children (before starting school)
* children who have commenced school up to age 14
* young people aged 15-24
* adults aged 25 and over

If you don’t respond to this request TODAY it’s going to become a deemed refusal and I’m going to be extremely disappointed and further disillusioned about the whole freedom of information process and democracy in general, because you’ve some how let this happen again. I’m tired. PLEASE RESPOND.

May I please clarify one thing –
There are no version numbers and dates recorded alongside the questionnaires. Can you confirm that these are published and in use with participants at the date of my FOI request (March 2nd 2024)? Particularly the family questionnaires (document 5-7) which are marked as "draft".
