Good morning,
Please find attached a Freedom of Information (FOI) application form to be completed and within the scope of the request the more precise you can be, the better we can administer the search for the records required.
The application will attract a fee of $30.00 which can be remitted by providing a credit card number with authority to debit the amount of the fee. This can be sent electronically to me and I will action the rest.
Proof of photo identity such as a copy of your driver’s license or passport which has been confirmed a true copy by a Justice of the Peace or any one with authority to sign the document, once this has been received the Freedom of information action will be commenced.
You will be notified by an acceptance letter of the request and further notified of any stages in the procedure as the FOI progresses. For more information you can visit the site of the Information Commissioner of the NT - http://www.infocomm.nt.gov.au/
