information for individuals. There is also no central database for medical
records across NSW.

Unfortunately, there is no central database for medical records, and medical records/health records are not held by the Ministry of Health.

Health records are held by the hospital medical records department or private facility (e.g. hospital, health facility) where you received care.

As the Prince Henry Hospital facility closed in 1988, you will need to reach out to the Sydney Local Health District for further information. A link to their website can be found here: https://slhd.health.nsw.gov.au/
You do not need to submit a GIPA application for this information, this information can be accessed under the Health Records Information and Privacy Act (HRIP Act) which is the appropriate legislation to obtain personal health information.
