Dear Australian Prudential Regulation Authority,
The document I seek is a copy of a document with the heading
"ONLINE SUPERANNUATION BREACH NOTIFICATION FORM"
The document is the blank template of the form that should be lodged with APRA by a trustee of a regulated superannuation fund in the event of a breach of Trustee Covenants pursuant to Section 52 and 52A of the Superannuation Industry (Supervision) Act 1993.
