Dear Ms Ley and the Department of Health,

I am writing to you under the Freedom of Information Act 1982.

I request copies of documents and or reports which identity the dollar cost to Medicare for the number of revision operations for hips and knees replacements which were caused by recalls/hazards and failures of those items. Time period: From December 2005 to December 2016.

I made an FOI request, asking for the number of operations covered by Medicare only for Revision of failed hips and knees.
By law, I should have received a prompt response by May 19, 2016.
I have not received the above request in the required reasonable time,
i therefore request an internal review.
