Information surrounding a public safety incident 16/03/2016 by Westconnex contractors at their Cintra site
Dear WorkCover Authority of NSW,
I want to see the incident report 1-356252
I want to see the determinations made by Workcover regarding the safety breaches.
I want to see what safety instructions Workcover determined the contractors should take when completing this work.
I want to see what penalty if any that Workcover advised the contractors would be enforced should proper safety procedures not be adhered to by them.

To seek access to information held about this matter, please complete a
Government Information (Public Access[1]) application form available on
our [2]website. SafeWork charge an application fee of $30 and, where
applicable, processing fees may also apply. An advance deposit of a
processing charge may be required. If the processing time exceeds 20
hours, charges of $30 per hour will apply. A concession rate of $15 per
hour may be considered in some circumstances. where an applicant can
certify the following:
• the applicant holds a Pensioner Concession Card, or
• is a full-time student, or
• represents a non-profit organisation, or
• is able to demonstrate financial hardship, or
• is able to show that the information they seek is in the
public interest.
