REQUEST FOR INFORMATION
I am wiring to you to as to request information through the Freedom of Information Act 1992 (WA) (FOI Act).
I request specific documents such as the Original Public Health Assessment in relation to "Advice on Liquor Stores & Bottleshops" Vaccine Mandates.
As well as the Public Health Re-Assessment of the Risk Benefit Achieved which supported the change to Vaccine Mandates for Liquor Stores & Bottleshops
These relate to the decision made by the WA Government under the Guidance of CHO on 22th February 2022.

for information below.
You have requested the document that relates to each one of the following
points:
o Public Health Assessment for the validity of mask mandates for
children aged 5-11 years
In relation to the Chief Health Officer’s advice on 22 February 2022:
o Original Public Health Assessment in relation to advice on liquor
stores and bottle shops vaccine mandates
o Public Health Re-assessment of the risk benefit achieved which
supported the change to vaccine mandates for liquor stores and bottle
shops
Can you please confirm your agreement with the above scope.
Secondly, in order for an invoice to be raised enabling online payment of
the $30.00 FOI application fee, it is a mandatory requirement that a
contact telephone number be provided.
