All workplace health and safety assessments containing:

Ventilation assessments conducted following outbreaks

If you are seeking access to information under the Right to Information Act 2009 (the RTI Act), you must make an application directly to the Agency, and it must also satisfy the following requirements:

• Be on the approved form.
• Be accompanied by the application fee.
• Give sufficient information concerning the document to enable an officer of the agency to identify it.
• State whether or not the applicant is accessing the documents to benefit another entity (the beneficiary) and, if they are, identify the beneficiary.
• Include evidence of ID, and if the applicant has an agent, include evidence of the agent's ID and evidence of authority, only if the application is for the applicant's personal information.
• State an address to which notices can be sent.

The best way to submit an application under the RTI Act is via the Queensland Government online portal, at www.rti.qld.gov.au. This method will allow you to pay the mandatory application fee via credit card when you submit the application.
