Thank you for your email and preliminary FOI application.

In order to assist you further, I will need to obtain some further
information from you.

Under the FOI Act, an application for access to an agency’s document –

 a. Must be in writing; and
 b. Must specify that it is made under this Act; and
 c. Must be accompanied by such application fee as may be prescribed; and
 d. Must contain such information as is reasonably necessary to enable the
documents to be identified; and
 e. Must specify an address in Australia to which notices under this Act
should be sent; and
 f. Must be lodged at an office of the agency, and may request that access
to the document be given in a particular way.

You can either complete the attached FOI form which will capture all the
required information as above, or you can reply to this email confirming
that this request is being made under FOI, how you would like to receive
the documents eg in hard copy, electronically via email) and provide an
address in Australia where notices can be sent. Please also advise whether
there is a different email address you would like to use for
correspondence or whether this one is suitable.

There is a $37.50 application fee applicable. Payment can be made in
person at the Civic Centre, the Greenacres Library Council Office or
