Please provide the following:

- Clinical guidelines for the assessment, diagnosis, treatment, and rehabilitation protocol of proximal hamstring injuries of all degrees of presentation, from April 1st 2019 through to present

- Criteria for MRI for proximal hamstring injuries, both in absence of USS results AND in presence of USS results that amount to being inconclusive, from April 1st 2019 to present

When doing so please kindly:

- Specifically mention any/all excluding criteria/justification for refusing the provision of MRI for proximal hamstring injuries of all respective degrees

- Provide any indications of changes that have been made to any of the above since April 1st 2019
