Dear Transport Accident Commission Victoria,

I would like a copy of all medical records from my initial admission to St Vincent Hospital, Melbourne, all subsequent medical reports, and all records, notes, copies of files and internal notes made with respect to my claim number 88/03475.

This must include all notes classed as "For Internal Only" purposes, as well as any confidential notes between assessors and any Doctors that provided any assessments regarding the claim.

I would also like a copy of all records in relation to the automobile accident at Clarke Street, Northcote, that occurred with respect to the claim number above.

Also included, must be any reports made about any scans, x-rays and initial medical assessments made in the St Vincent ER.

In my last FOI request, I was denied part of my request, with an internal memo stating that to provide this, I would have the right to a civil law suit. If any memo's or notes are not provided this time, I will file a subsequent FOI request, or refer the matter to the Administrative Appeals Tribunal of Victoria.
