Hi Bruce,
Thanks for getting in touch and for trying to keep your request public and available to all on Right To Know. You cannot attach a PDF to requests on Right To Know (you shouldn't have to, so we don't have a function for it) and as that form contains a lot of personal information you probably wouldn't want it in public either. But we'll try to work out a way through this.
I just called the NSW Information and Privacy Commission (1800 472 679) and spoke to Kristina. She advised that if you want to make an informal request then there is no requirement to fill in the form (you can even make them over the phone!). Strictly speaking there is also no requirement to fill out a form for a formal request but she said it helps by ensuring your request is valid under s41 of the Act (http://www.legislation.nsw.gov.au/fragvi...).
There are trade-offs either way. If you make an informal request you basically get none of the rights that the law would otherwise provide you, such as a defined timeframe for a response (i.e. they can just ignore you). However if you choose to make a formal request it's a $30 application fee before you even start.
I think the best thing to do is to reply to the council and ask if you can make this an informal request via email. If they decide not to help then you can go down the formal request path.
If you decide to do that you can still ask them to reply to your request using this righttoknow.org.au email addres which will mean the response will appear here for everyone to benefit from.
