9 Colsoun Rd Ashfield building inspection request
Dear Town of Bassendean,
I would like to make a request to your environmental safety office for information regarding the property located at 9 Colstoun Rd Ashfield.
I was a tenant at this property between October 2016 and April 2018. I experienced physical and mental health issues due to breathing in toxic mould spores that occurred as a result of a mould infestation. The mould infestation appeared to be caused by the structural water damage to the building.
I would like to request all documents relating the inspection and subsequent rulings regards the habitability of 9 Colstoun rd Ashfield carried out during the months of April and May in 2018. This includes electronic documents, emails, written notes and request tickets.
Yours faithfully,
Christopher P. Caston
