I request a full unedited copy of the external review into the Road Policing Service Delivery Model undertaken in 2016 and delivered to Executive Command in 2017.

I also request any notes made throughout the review process and any / all notes made throughout the decision-making process to reject the 16 recommendations made by the review.

Please note the request will need to be accompanied by a copy of your photo ID, and payment of the $30.60 application fee or evidence for a fee waiver (e.g. Health care card).
 
If you choose to submit your request by mail then payment is required by either money order or cheque.
 
A request needs to provide our office with enough information to identify the documents being requested. As such your request should include:
 
• the nature of the incident or investigation
• Victoria Police’s involvement
• the date and location of the incident
• the names and dates of birth of any other people involved
• the names of any investigating police members
• the police station involved.
 
Personal affairs information about third parties is generally exempt from release under the Freedom of Information act and I recommend, where possible, obtaining written authority from the third party allowing us to release their information to you. If the third party is deceased then we will require authority from their senior next of kin.

Please also note that in accordance with section 67 of the Freedom of Information Act, an applicant is unable to obtain records created prior to 1977 unless the documents specifically relate to themselves.
