Dear Queensland Metro South Health,

Please provide the approval in accordance with the Commonwealth Privacy Act or My Health Records Act 2012 or My Health Records Regulation 2012 or My Health Records Rules 2016 to access the commonwealth E-health records of persons notified to the department as returning a positive HIV result in a Queensland Lab.

This request in particularly request approvals for the team that responds to the following email address - xxxxxxxxxxx@xxxxxx.xxx.xxx.xx and the Postal Address Locked Bag 28, FORTITUDE VALLEY BC QLD 4006.

It is believed that the Manager of this Team is Ms Jacqueline Kennedy - Public Health - HIV (Team Leader) - Communicable Diseases Breach, Prevention Division.
