Subject: Freedom of Information request - COVID-19 Vaccine Injections on HMAS Sydney

Dear Department of Defence,

Around 10 March 2021 media reports revealed that around 80% of the crew of HMAS Sydney had reported ill after receiving an injection of a COVID-19 vaccine.

The documents I seek are documents that:
(i) would confirm the date on which these injections were given;
(ii) would confirm whether these injections are compulsory for navy personnel;
(iii) the type of vaccine used (ie Pfizer or AstraZeneca); and
(iv) how many sailors were transferred to the hospital for treatment?
