The agency can receive several reports about a single incident from different sources (e.g. Provider and family member) this means the total number of reports submitted is not a direct correlation to the number of incidents that occurred.
The use of < within the table is to maintain and protect confidentiality and prevent the potential identification of individuals which can occur if the data were combined with other available information.
