Dear Endeavour Energy,

I would like to make a FOI request regarding the report and all documentation regarding the disruption to essential services (electricity) at the address of Unit 12/14 Sorrell Street, Parramatta, NSW 2150 in the year 2021.

I am requesting all documentation associated to this matter including:

- The reason for the supply being disrupted
- The fault finding and repair
- The date and time the disruption was reported as well as, the date and time it was resolved
- How the issue was first brought to attention
- The communication between Endeavour Energy and the retailer AGL (including emails and reports)
- The process Endeavour Energy took to evaluate, inspect and fix the disruption.
- The offical Endeavour Energy report about this matter
